5 – CIRCUMSTANCES OF THE ACCIDENT: You must indicate, through the system of crosses, the circumstances in which the accident occurred. Don’t forget to indicate the number of squares indicated at the end. This information is very important for determining responsibilities.
6 – INSURED: Indicate the Surname, Name and Address of the insured.
7 – VEHICLE: Indicate the registration number of the vehicle.
8 – INSURANCE COMPANY: Indicate the Insurance Company according to the vehicle’s Green Card.
9 – DRIVER: Indicate the elements that appear on the driver’s driving license.
10 – VISIBLE DAMAGE: Record the visible damage caused by the accident in question.
11 – OBSERVATIONS: Indique, resumidamente, as circunstâncias do acidente (exemplo: o veículo B tinha perda de prioridade).
12 – ACCIDENT DIAGRAM: It is important to indicate the position of vehicles at the time of collision, as well as the existence of traffic lights, vertical signs, continuous traces and other reference elements and or signaling.
13 – DRIVERS’ SIGNATURE: Make sure that the declaration is signed by both drivers, thus ensuring the possibility of triggering the IDS system if the situation permits.
